About the Online Directory of Health & Social Care Services Project

The Partnerships for Access to Health (PATH) project is about helping people with multiple and complex needs to access, engage with and get the most from the services they need.  It’s focus is on helping those with multiple and complex health and social problems, and a key part is developing partnerships between NHS, Social Care, Voluntary agencies and other partners. The Mid Highland Community Health Partnership is one of four pilot sites across NHS Scotland, and Skye and Lochalsh is taking forward the first stage. The directory is one of the early outcomes of this project.

Self care has been highlighted as one of the key building blocks for a patient-centered health service, and helping people self care represents an exciting opportunity and challenge for the NHS and social care services to empower patients to take more control over their lives. There are a lot of health and social care services offered by the voluntary sector locally, and access to information about these local services is important for both practitioners and patients. The directory aims to provide this information in an easily accessible format. Statutory services are not included in the directory as information about them is best obtained from the parent bodies.
